JFK Memorial Hospital has provided medical care to residents in the Coachella Valley since 1966. A group of physicians, including Dr. Reynaldo Carreon, founded the hospital and today one of JFK�s cross streets is named in his honor.

POSITION DESCRIPTION:
The Care Coordinator (CC) is responsible for facilitating a smooth and patient-centered admission and post hospital discharge process for JFK Memorial Hospital patients ensuring appropriate level of care to help patients achieve optimal health and access to care to prevent avoidable readmissions. The CC will use critical thinking skills to expedite admissions and effective discharge processes at JFK Memorial Hospital while demonstrating JFK�s Mission, Vision, Values and Service Standards in all interactions with referring facilities, physicians, employees and guests.POSITION SPECIFIC RESPONSIBILITIES� Coordinates and facilitates direct admissions through the �EZ Admit� process� Coordinates and facilitates transfer admission of patients from other healthcare facilities to JFK using JFK�s �EZ Admit� process in accordance with policies and procedures� Collaborates with JFK on-call physicians, hospitalists admitting physicians or designee to arrange accepting physician.� Communicates with referring physician, hospital, prison, or other alternative level of care or provider to coordinate transportation arrangements and secure an assigned bed and account number, as well as, assign an Ambassador.� Collects clinical information to verify the patient�s level of care matches the type of bed ordered by the physician and that the level of care being requested is medically appropriate� Assess successful transition of care to the patients home by providing follow up phone calls to:� o Ensure that patients are able to make their scheduled appointments and if not, rescheduling it for them.o Ensure post hospital services are in place, as ordered by the case managerment team, in an aim to prevent readmissions.o Clarify any confusion over the patients medications. o � Runs daily readmission report to identify patients appropriate for post hospital follow up call interventions� Redesigns the patient education process to improve patient/family/caregiver understanding of self-care� Attends TEMPO and updates TEMPO Board with patients readmitted� Assures timely follow up of patient, family, staff and physician concerns� � Maintains competence in all computer systems� Maintains 90% accuracy with all information entered in computer system� Participates in process improvement activities� Self-directed and able to work with minimal supervision while remaining flexible and cooperative in fulfilling role obligation� Influences balance scorecard metrics by increasing admissions through direct admits and transfers, as well as ensuring quality metrics and initiatives are met.� Maintains a good working rapport with all staff inter and interdepartmentally which may require frequent interactions and coordination of efforts� Demonstrates problem solving skills� Develops action plans as needed to achieve process improvement� Demonstrates effective and responsible relationships with patients, families, medical director and key personnel such as physicians, employees, management, and administration and related outside agencies. Communicates any problems immediately to the Director or Case Management or House Supervisor. Qualifications:
QUALIFICATIONS:Required:� Active Registered Nurse with a California license. Or Social Worker� Minimum of 3-5 years ER/ICU/telemetry or medical/surgical acute hospital experience.� Bachelor of Science in Nursing or equivalent� Demonstrated organizational skills, excellent verbal and written communication skills and excellent phone etiquette.� Ability to manage multiple priorities.� Ability to manage a specific population of patients identified at risk� Demonstrated ability to present self in positive and professional manner.� Proven critical thinking and problem solving skills.� Ability to lead and coordinate activities of a diverse group of people in a fast-paced environment.� Computer literate.� Current American Heart Association BLS CertificationPreferred:� 3 - 5 years of Case Management experience, knowledge of McKesson�s Interqual Criteria
